Job summary

Zion's Way Home Health & Hospice in Utah seeks a compassionate hospice aide under RN supervision to perform personal care, monitor patient status, and support comfort in home settings.

You will observe and document patient care, assist with daily living activities, and communicate effectively with patients and families while maintaining confidentiality and safety. A valid license and CPR are required.

Qualifications

• Meets state training requirements and completes 75 hours of classroom and 16 hours of practical training.
• At least 18 years old.
• Ability to read and follow written instructions and document care.
• Current CPR certification.
• Licensed driver with insured vehicle and good driving record.

Responsibilities

• Perform personal care tasks after patient assignment and address requests promptly.
• Report to Case Manager if unable to perform a task.
• Provide patient and family with respectful communication and maintain confidentiality.
• Assist with meals, feeding, and hygiene.
• Monitor vital signs when ordered and record observations.
• Assist with ambulation and transfers as directed.
• Maintain patient safety and prevent skin breakdown.
• Keep living area clean and organized.
• Adhere to documentation and care procedures.
• Participate in quality assessment and improvement activities.
• Respect patient and family environment.
