Job summary

CNHHS Hospice in Tahlequah, OK seeks a Certified Home Health Aide to join our compassionate team caring for terminally ill patients at home. You will work under an RN, providing personal care and supporting families through bereavement.

The role requires a CHHA, high school education, and at least two years of nursing assistant experience. You will deliver respectful, patient-centered care following the care plan and OSHA guidelines.

Qualifications

• High school diploma or GED required.
• Graduate from an approved training and competency course.
• Certified Home Health Aide (CHHA) credential required.

Responsibilities

• Provide essential personal care to hospice patient under RN supervision.
• Bathing, mouth care, skin and hair care.
• Assist with bathroom needs and transfers as ordered.
• Prepare meals with special diets.
• Take vital signs as assigned by RN.
• Offer emotional support to patient and family.
• Document supervision and evaluations every 14 days.
• Attend 12 hours of inservice education per year.
