Job summary

Grace Hospice is seeking a compassionate Hospice Aide to provide in-home, patient-centered care in Cincinnati and surrounding communities. You will support patients and families, maintain dignity and comfort, and follow each plan of care with respect and professionalism.

Applicants must be a certified nurse’s aide (CNA) or STNA in Ohio, hold a valid driver’s license, and be able to work within a multidisciplinary team during field visits. A high school diploma or GED is preferred.

Qualifications

• Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio.
• Must maintain a valid driver’s license, good driving record and reliable automobile.
• High school graduates or GED preferred.
• Demonstrate the ability to be able to work within a multi-disciplinary team setting.
• Demonstrate the ability to work in field situations.

Responsibilities

• Provide compassionate personal care and supportive services to patients in their place of residence.
• Deliver care in alignment with the patient’s individualized Plan of Care.
• Follow the Plan of Care and update during scheduled visits.
• Support patients with activities of daily living (ADLs) and mobility as needed.
