Job summary

Longwood Hospice seeks a Hospice Aide to provide compassionate, hands-on personal care in patients' homes or care facilities under RN supervision. Duties include bathing, transfers, vital signs, documentation in MatrixCare, and adherence to the care plan.

The role emphasizes patient comfort, dignity, and emotional support, with flexibility for evenings or weekends as needed. A MA CNA/HHA/Hospice Aide training and a valid driver’s license are required.

Qualifications

• High school diploma or equivalent.
• Completion of MA Certified Nurse Aide, HHA or Hospice Aide training.
• Valid driver's license and reliable transportation for home hospice care.

Responsibilities

• Assist patients with personal care including bathing, grooming, dressing, toileting, and oral hygiene.
• Help patients with mobility, transfers, positioning, and use of assistive devices.
• Take and record vital signs as directed.
• Observe and report changes in patient condition to the Hospice nurse.
• Provide skin care and help prevent pressure injuries.
• Assist with light housekeeping related to patient care.
• Support patient comfort and emotional well-being with empathy and respect.
• Follow the established hospice care plan and Longwood Hospice policies.
• Maintain accurate documentation of care provided in MatrixCare.
• Practice infection control and safety procedures at all times.
• Respect patient privacy, confidentiality, and cultural preferences.
