Care Initiatives Hospice seeks a compassionate Hospice Aide to support patients and families in their chosen setting, including home, long-term care communities, assisted living, or hospital. You will provide personalized care, emotional support, and engage patients in activities to improve quality of life during life-limiting illness.

The role emphasizes dignity, respect, and collaboration with families and care teams.

Qualifications

• Experience as Hospice Aide, Home Health Aide, or CNA in the last 24 months preferred.
• Hospice or palliative care experience preferred.
• Active CPR certification required.
• Current driver’s license and ability to travel between locations.
• Plan to obtain Hospice Nursing Assistant Certification (CHPN) within 24 months of employment.

Responsibilities

• Assist with daily living activities (bathing, dressing, grooming, eating).
• Provide emotional support and companionship to patients and families.
• Communicate effectively with residents, families, and healthcare team.
• Encourage participation in recreational and therapeutic activities.
• Follow infection control protocols to maintain a safe environment.
