Job summary

Sarah Bush Lincoln Health Center seeks a Hospice Aide/CNA (Hospice House) for 6am-6pm PRN shifts to support compassionate end-of-life care. You will assist with activities of daily living, follow the patient care plan, and work under an RN to ensure comfort and safety in a hospice setting.

This role requires a High School diploma, CNA certification or progress in RN studies, and a Illinois driver’s license. On-the-job training, ongoing education, and a supportive team culture are offered.

Qualifications

• High School diploma or equivalent required.
• CNA certification or current completion of the first RN semester is acceptable (Sarah Bush Lincoln).
• Valid driver's license (Illinois).

Responsibilities

• Assist in implementing the patient Plan of Care.
• Assist patients with activities of daily living.
• Attend in-services and staff meetings; pursue continuing education.
• Follow directions from the RN for the assigned patient.
• Maintain a clean, safe environment and perform light housekeeping as assigned.
• Document observations and tasks completed or refused.
• Observe and report significant changes in patient status to supervisor.
• Participate in interdisciplinary team meetings.
• Provide hands-on personal care and support to hospice patients.
