Heart to Heart Hospice in Michigan is hiring a Hospice Aide (CNA) to provide direct patient care under the supervision of an RN and per the Plan of Care. You will assist with transfers, ensure safety, and communicate changes to the care team.

Applicants should have an unencumbered CNA license, a high school diploma or GED, and at least one year of experience in hospice or long-term care. A valid drivers license and reliable transportation are required.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification.
• Current CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health, hospice or long-term care facility.
• Demonstrates interest in welfare of ill and elderly; maintains confidentiality.
• EMR experience a plus.
• Must have a valid drivers license, reliable transportation and ongoing auto liability insurance.

Responsabilités

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide skills to assist the patient with safe transfers and ambulation per agency policy.
• Appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, respectful communication to patient/family and other employees.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
