Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of the RN in our Michigan-based team. You will assist with transfers, comfort measures, and daily activities according to the Plan of Care.

Preferred candidates hold a CNA license, a high school diploma or GED, and at least one year of hospice or long-term care experience; EMR experience is a plus. We offer competitive pay, benefits, and opportunities for growth.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license per Nurse Aide Registry.
• Current CNA license required.
• At least 1 year experience as Hospice Aide or Nursing Assistant in healthcare settings.
• Demonstrates interest in welfare of ill and elderly; compassionate care.

Responsibilities

• Provide direct patient care to patients under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient, family, and staff.
• Submit documentation timely.
• Perform other duties as required.
• Ensure compliance with laws and policies.
