Job summary

Heart to Heart Hospice in Michigan is seeking a compassionate Hospice Aide/CNA to provide direct patient care under the supervision of the RN and the Plan of Care. You will assist with transfers, maintain infection control, and document care to ensure continuity of services.

The role requires an unencumbered CNA license, a high school diploma or GED, and at least one year of hospice or nursing assistant exposure.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA certification/licensure.
• CNA license currently active and in good standing.
• At least 1 year of Hospice Aide or Nursing Assistant experience.
• Interest in welfare of ill and elderly; compassionate care mindset.
• High integrity and confidentiality; EMR experience a plus.
• Valid driver’s license and reliable transportation with insurance.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided for continuity.
• Practice infection control principles.
• Maintain respectful communication with patient, family, and staff.
• Timely submission of all documentation.
• Perform other duties as required; ensure compliance with laws and policies.
