Job summary

Optum is hiring a Hospice Aide/CNA to support patients at home within the Heart of Hospice program. The role focuses on personal care, daily living activities, and medication reminders to help clients remain in their own homes. You will document conditions, manage vital signs, and ensure a safe home environment.

The position requires CNA certification or completion of a training program, with CPR/First Aid preferred. Competitive hourly pay and comprehensive benefits are offered.

Qualifications

• Provide personal care and assist with daily living activities including bathing, grooming, dressing, ambulation, and medication reminders.
• Monitor client condition and document vital signs and incidents with confidentiality and EVV.
• Ensure safe home environment and correct use of medical equipment.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
