Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of a registered nurse. You will assist with transfers, report changes, and document care to ensure continuity of care while adhering to infection control principles.

The role requires a current CNA license, at least one year of related experience, and a driving license with reliable transportation. Competitive benefits are provided with full-time employment.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA certification per Nurse Aide Registry.
• Current CNA license.
• At least 1 year as Hospice Aide or Nursing Assistant.
• Demonstrates interest in welfare of ill and elderly; compassionate care.
• High integrity and confidentiality of information.
• EMR experience is a plus.
• Valid driver's license, reliable transportation, and auto liability insurance.

Responsibilities

• Provide direct patient care under RN direction according to Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care provided for continuity of care.
• Apply infection control principles.
• Promote positive, respectful communication with patient, family and staff.
• Submit documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and personnel policies.
