Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to join our team.

You will work under the supervision of a Registered Nurse to provide direct support and personal care for hospice patients in a home setting, following each patient’s Plan of Care and coordinating with the RN Case Manager. Responsibilities include bathing, mobility assistance, meal support, and documentation while maintaining HIPAA standards.

Qualifications

• Active Certified Nursing Assistant (CNA) license in the state of practice.
• Must be at least 18 years old.
• Ability to pass background, consumer report and drug test.
• Valid driver’s license and reliable vehicle for home visits.

Responsibilities

• Provide personal care including bathing, dressing, oral hygiene, and bed linen changes.
• Assist patients with mobility, transfers, and exercises per nursing staff.
• Support meals, feeding, and light household tasks as assigned.
• Monitor patient condition changes and report to RN Case Manager.
• Maintain documentation and coordinate with care team while ensuring HIPAA compliance.
