Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide to join our interdisciplinary care team. Work under RN supervision to provide high-quality, timely patient care and precise documentation of care.

This role emphasizes 1-1 patient interaction, safety, and dignity in end-of-life support. You will assist with personal care, vital signs, medications assistance, nutrition, mobility, and family communication while maintaining HIPAA privacy and professional standards.

Qualifications

• Must be 18 years of age.
• Active CNA certification current with state-specific requirements.
• Licensed and insured driver with automobile in good working order.
• Ability to maintain HIPAA privacy standards and patient confidentiality.

Responsibilities

• Provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene.
• Maintain a sanitary and orderly environment.
• Communicate respectfully with patients and families.
• Record vital signs and assist with self-administered medications.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Assess patient condition and report changes to the RN Case Manager.
• Assist in ambulation, transfers, positioning and range of motion activities.
