Job summary

Northern Arizona Hospice, part of the Optum family, seeks a Home and Community Based Services Aide to provide in-home personal care and support for clients to remain in their own homes. You will assist with daily living activities, monitor conditions, and help with household tasks while using EVV and maintaining confidentiality.

The role emphasizes flexible hours, independence, and delivering compassionate care.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
