Qualifications

• High school diploma or equivalent required.
• 1–2 years healthcare intake or related experience required.
• Hospice intake or referral coordination experience preferred.
• Knowledge of insurance verification and Medicare/Medicaid guidelines preferred.
• Experience working with EMR systems.
• Strong organizational skills with ability to manage multiple referrals and priorities.
• Excellent written and verbal communication skills.
• Compassionate and professional when working with patients and families.
• Strong attention to detail and follow-through.

Responsibilities

• Receive and process new hospice referrals from hospitals, physicians, facilities, families, and other referral sources.
• Gather required patient information and documentation to support timely admissions.
• Communicate with patients and families regarding hospice services and the admission process.
• Coordinate referrals and admissions with nurses, social workers, providers, and other members of the hospice team.
• Verify insurance coverage, benefits, and eligibility for hospice services.
• Accurately enter and maintain patient information in the EMR.
• Track referrals and follow up on outstanding documentation or information.
• Maintain communication with referral sources and provide timely status updates.
• Complete intake and admission documentation accurately and efficiently.
• Ensure intake processes comply with Medicare, Medicaid, state, and organizational requirements.
• Protect patient confidentiality and follow all HIPAA requirements.
