Job summary

Focus Healthcare Solutions seeks a Homecare Navigator to serve as the primary relationship manager for clients from inquiry through ongoing care. You will manage intake, onboarding, and ongoing contact, building trust with families and coordinating with nurses when needed.

In this field-based role, you will support scheduling, caregiver matching, and timely start of care while maintaining strong community partnerships and ensuring adherence to care plans.

Qualifications

• Excellent relationship-building, communication, and conflict-resolution skills.
• Background in intakes, assessments, or care plan development strongly preferred.
• Experience with Medicaid waiver programs, service coordinators, older adults, or family caregivers a plus.
• Valid driver's license, reliable transportation, and willingness to travel throughout the service area.

Responsibilities

• Serve as the first contact for new client inquiries and conduct in-home, non-clinical assessments.
• Determine when a case needs escalation to a nurse and route accordingly.
• Build accurate care plans and guide clients through intake documentation and next steps.
• Guide onboarding, set expectations for scheduling and service, and support a strong caregiver match and timely start of care.
• Maintain regular visits and check-ins to retain clients and address concerns.
• Verify caregivers follow the care plan through home visits and service reviews.
• Build relationships with state coordinators and referral partners.
