VitalCaring Group is seeking a field-based Care Transition Navigator to ensure safe, seamless hospital-to-home health transitions. You will collaborate with hospital teams, physicians, and families to coordinate care and reduce readmissions.

This role blends clinical insight with care coordination and referral management to support patient outcomes and agency growth across our home health network.

Formación

• Active RN, LVN/LPN, or PT license in the state of employment.
• Minimum of two years of clinical experience; home health or post-acute experience preferred.
• Experience in healthcare coordination, case management, or hospital-based roles.
• Proficiency with EMR systems and basic computer applications.

Responsabilidades

• Serve as liaison between hospital teams, patients, and VitalCaring clinicians for seamless transitions.
• Conduct bedside assessments to identify needs and barriers to discharge.
• Develop and execute safe, patient-centered transition plans with case managers and physicians.
• Coordinate referrals to drive timely admissions into home health services.
• Build trusted relationships with hospital partners through consistent communication.
• Complete post-discharge follow-up within 48 hours and coordinate primary care.
