Job summary

Signify Health, part of CVS Health®, seeks a Part Time Physician to provide In-Home Health Evaluations, visiting patients at their homes and bridging gaps in care. You’ll build trusted relationships and deliver flexible, comprehensive health evaluations right to people’s front doors.

The role includes a 45-minute visit, medical history review, physical assessment, and potential diagnostic tests when requested by the health plan. Travel within licensed states is expected.

Qualifications

• Active, unrestricted medical license in the applicable state(s).
• Multi-state licenses preferred.

Responsibilities

• Engage with patients during in-home evaluations to review health history and med usage.
• Perform physical assessments and, when ordered, basic diagnostic tests.
• Travel locally to patients' homes within licensed states as needed.
• Work with Signify Health clinicians to deliver Diagnostics & Preventive Services.
