Job summary

TEEMA Solutions Group is seeking a compassionate Speech Language Pathologist (SLP) to provide home health speech therapy services. The SLP will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders while developing individualized treatment plans to improve patients' communication, swallowing function, and overall quality of life.

The role requires a Master’s degree in Speech-Language Pathology, active SLP license, and BLS/CPR certification, with ASHA CCC

Qualifications

Master's Degree in Speech-Language Pathology or equivalent.
Active SLP license in the state of practice.
Current BLS/CPR Certification.
ASHA CCC is preferred.
Valid Driver's License and reliable transportation.
Strong communication, documentation, and patient education skills.
Home health or clinical experience preferred.

Responsibilities

Evaluate patients with speech, language, cognitive, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs.
Provide speech therapy interventions to improve communication and swallowing abilities.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients, families, and caregivers on therapy techniques and home exercise programs.
Maintain accurate, timely, and compliant clinical documentation.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
Ensure compliance with all applicable healthcare regulations, safety standards, and organizational policies.
Perform other job-related duties as assigned.
