Job summary

Providence at Home with Compassus seeks a Speech-Language Pathologist to plan and provide treatment for patients with communication disorders in a home-health setting. You will assess, document, and design a measurable care plan while coordinating with families and providers.

Responsibilities include timely assessments, data documentation, care coordination, and ensuring Medicare/Medicaid compliance. CA license, a Master's in SLP, and BLS certification within 30 days are required.

Qualifications

• Required: Master's Degree in Speech-Language Pathology or equivalent.
• Required: California Speech-Language Pathologist license.
• Required within 30 days: National Provider BLS - American Heart Association.
• Preferred: 1 year clinical experience as a Speech Pathologist.

Responsibilities

• Assess SLP care needs after referral and perform history and physical examination.
• Document assessment data within 24 hours of home visit and develop a written plan of care.
• Coordinate care, referrals, and ensure Medicare/Medicaid compliance and reimbursement eligibility.
• Communicate with insurers to determine eligibility for home health services and plan patient safety.
• Deliver treatment interventions as per the care plan and supervise related staff as needed.
• Complete discharge summaries and maintain accurate, HIPAA-compliant documentation.
