Signature Eugene Home Health is seeking a compassionate Medical Social Worker (LCSW) for full-time field-based work in Eugene, OR. You will provide psychosocial support, care coordination, and community resource guidance to patients and families navigating transitions.

The role emphasizes advocacy, teamwork with an interdisciplinary home health team, and commitment to quality improvement within hospice/home health services.

Qualifications

• Licensed in Social Work (LCSW) from an accredited program.
• 1+ year of healthcare experience (hospice or home health preferred).
• Strong communication, organization, and interpersonal skills.
• Ability to work independently in a home health/field-based setting.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Develop and implement care plans based on social and emotional needs.
• Provide grief counseling, bereavement support, and crisis intervention.
• Coordinate community resources, referrals, and support services.
• Serve as a liaison between patients, families, and healthcare providers.
• Collaborate with the interdisciplinary home health care team (RN, physician, aides).
• Participate in quality assurance and performance improvement initiatives.
