Job summary

Life Care Home Health is seeking a registered nurse to administer home health nursing services and coordinate care for clients in their residences. The role requires completing initial and ongoing assessments and developing plans of care to maintain quality outcomes.

Applicants should have an accredited RN program credential, an active RN license, CPR certification, and ideally one year of home health experience. A flexible schedule and on-call duties are expected.

Qualifications

• Current RN license in good standing; valid CPR.
• One year RN experience in skilled home health is preferred.
• Completion of an accredited Registered Nursing Program.

Responsibilities

• Administer nursing care in clients' residences and coordinate with the care team.
• Complete initial and ongoing assessments, including OASIS.
• Develop and revise the Plan of Care to ensure quality and continuity.
• Communicate effectively with clients, families, and health professionals.
• Supervise and provide guidance to other nursing staff as needed.
• Maintain timely documentation and comply with policies.
• Participate in on-call coverage and in-service programs.
