Qualifications

Responsibilities

• Promote the company's mission, vision, and values.
• Complete initial and ongoing nursing assessments including OASIS.
• Reevaluate nursing needs and outcomes; aid recovery and maintenance of life functions.
• Develop, initiate, and revise the Plan of Care for continuity of care.
• Provide communication with clients, families, and care team; participate in case conferences.
• Coordinate care with physicians and other personnel; inform changes in patient condition.
• Ensure documentation meets policy requirements and coordinates with third-party payers.
• Supervise and provide clinical direction to nursing staff.
• Perform on-call duties as assigned.
• Maintain ongoing professional development and continuing education.
