Altra Homecare Group in Indianapolis is seeking a dedicated Registered Nurse (RN) to deliver skilled home health services in patients' homes. You will ensure compliance with Indiana Medicaid requirements while partnering with patients and care teams to implement personalized care plans that promote health and independence.

Responsibilities include wound care, medication administration, chronic disease management, and education.

Qualifications

• Indiana RN license required and unrestricted.
• Minimum 1 year nursing experience; home health Medicaid preferred.
• Knowledge of Indiana Medicaid policies and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication and organizational skills.
• Familiarity with electronic documentation systems.

Responsabilités

• Provide skilled nursing services including wound care, med administration, chronic disease management, and patient education.
• Ensure services are medically necessary and properly documented for Medicaid reimbursement.
• Coordinate care with physicians, case managers, and the interdisciplinary team.
• Empower patients and caregivers with disease, med regimens, and self-care strategies.
• Maintain accurate, timely, compliant documentation in the EHR.
• Participate in Medicaid audits, QA initiatives, and case conferences.
• Connect patients with community resources and support services.
