Job summary

Qualifications

• Active Indiana RN license.

• Strong knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication and organizational skills.
• Familiarity with electronic documentation systems preferred.

Responsibilities

• Provide skilled nursing services in patients' homes, including wound care and medication administration.
• Ensure services are medically necessary and documented to meet Medicaid standards.
• Coordinate care with physicians, case managers, and the interdisciplinary team.
• Educate patients and caregivers on disease processes, medications, and self-care.
• Maintain accurate, timely documentation in the electronic health record (EHR).
• Participate in Medicaid audits, quality assurance initiatives, and case conferences.
• Connect patients with community resources and support services.
