Job summary

Altra Homecare Group is seeking a Registered Nurse (RN) for home health in Fishers, IN. The role centers on delivering skilled nursing care in patients’ homes while ensuring compliance with Indiana Medicaid requirements.

You will collaborate with physicians, case managers, and the care team to implement personalized care plans, educate patients and families, and document care within the EHR. Flexibility for evenings/weekends based on patient needs is possible.

Qualifications

• Current, unrestricted RN license in Indiana.
• Minimum of 1 year nursing experience, home health/Medicaid preferred.
• Solid understanding of Indiana Medicaid policies and documentation.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong clinical judgment with communication and organizational skills.
• Familiarity with electronic documentation systems.

Responsibilities

• Provide skilled nursing services, including wound care, med administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
