Job summary

Qualifications

• Current and unrestricted RN license in Indiana.

• Solid understanding of Indiana Medicaid policies and documentation.

Responsibilities

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Coordinate care with physicians, case managers, and interdisciplinary team.
• Document accurately in the EHR and participate in audits.
