Job summary

Altra Homecare Group in Indiana is seeking a dedicated Registered Nurse (RN) to deliver skilled home health services across patient homes, ensuring compliance with Indiana Medicaid requirements and high-quality, patient-centered care.

You will collaborate with physicians, care managers, and the inter-disciplinary team to implement personalized care plans, perform wound care, medication administration, and documentation in the EHR, with a flexible schedule including evenings or weekends.

Qualifications

• Current and unrestricted RN license in Indiana.
• At least 1 year of nursing experience; home health and Medicaid preferred.
• Solid understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication and organizational skills.
• Familiarity with electronic documentation systems preferred.

Responsibilities

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
