Job summary
Beth Israel Lahey Health is seeking a registered nurse for home health, delivering patient-centered care in Massachusetts. You will assess patients and families, initiate and revise care plans, and safeguard privacy while documenting care in our systems.
Requirements include an active MA RN license, a current BLS certification, and at least one year of direct care experience—preferably in home health or public health. OASIS/EPIC experience is a plus.
Qualifications
- Active Massachusetts RN license.
- Active BLS certification.
- At least 1 year of direct care nursing experience, preferably home health or public health.
- OASIS and EPIC experience preferred.
Responsibilities
- Complete and document initial patient and family assessment to determine home care needs.
- Initiate and revise the plan of care as patient status changes.
- Administer medications and treatments as prescribed by a physician.
- Instruct, supervise, and evaluate the home health aide care every two weeks.
- Identify discharge planning needs as part of the care plan development.
- Maintain HIPAA confidentiality and complete documentation with EPIC timely.
- Demonstrate proficiency in OASIS completion.
