Assisted Home Health Inc. seeks a compassionate Physical Therapist to provide in-home therapies in Sarasota, FL. The role requires a Florida-licensed PT with at least one year of healthcare experience and a valid driver’s license for up to 20% travel between patient locations.

You will assess, treat, and document patient progress, educate patients and families, and collaborate with the care team. A flexible schedule and strong charting discipline are expected.

Competenze

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by Florida.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet deadlines for charting and time management.

Mansioni

• Provide physical therapy as prescribed, in the home setting, and assist physicians with evaluation.
• Observe and record patient activities and findings in the clinical record.
• Report patient response to treatment and condition changes to the physician.
• Instruct patient and family in care and use of physical therapy devices.
• Instruct other health team members and family in relevant therapy phases.
• Supervise the Physical Therapy Assistant.
• Participate in case conferences, utilization reviews, orientations, and continuing education.
