• Maryland licensed Occupational Therapist.
• Minimum 1 year of OT clinical experience; home health preferred.
• Strong clinical judgment and clear communication with patients and families.
• Ability to travel to patient homes and maintain reliable transportation.

Responsibilities

• Provide skilled occupational therapy services in accordance with the physician’s plan of care.
• Implement therapeutic interventions designed to restore daily function and independence.
• Instruct patients and caregivers on strategies for daily activities, safe transfers, and positioning.
• Coordinate care with other healthcare professionals to ensure continuity and outcomes.
• Accurately document patient progress and submit records within 24 hours of each visit.
• Maintain compliance with agency policies, procedures, and home health best practices.
