Job summary

Infinity Hospice Care LLC is seeking a motivated Medical Social Worker to provide coordinated home-based care for clients across ages. You will conduct psychosocial assessments, referrals, and counseling to meet patient and family needs in a supportive, interdisciplinary team setting.

The role requires an MSW certification, a valid driver's license, and the ability to travel locally. You will collaborate with clinicians to promote optimal care and participate in ongoing education and

Qualifications

• Bachelor's or Master's degree in Social Work.
• Current MSW Certification in applicable states.
• Current driver's license and reliable transportation.
• Hepatitis profile if required for visits.

Responsibilities

• Assist in admission by evaluating psychosocial, financial, environmental, and community needs as evidenced by POC and records.
• Maintain knowledge of public and private eligibility standards.
• Collaborate with interdisciplinary team to coordinate patient care.
• Accept clinical assignments within education and competency.
• Provide effective communication to clients, families, and health professionals; participate in team meetings.
• Participate in in-service programs and ongoing competency development; complete required continuing education.
