Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsibilities

• Provide high quality clinical services within the scope defined by state law and plan of care.
• Document patient visits thoroughly in the EMR and adhere to regulatory and payer requirements.
• Participate in care coordination, report changes, and communicate with patients, families, and clinicians.
