Job summary

Qualifications

• Current LPN license (state-specific)
• Current CPR certification
• 1+ year of LPN or LVN experience

Responsibilities

• Verify plan of care with the RN prior to each visit
• Collaborate with physician and RN to meet patient needs; notify changes promptly
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review medications and perform medication reconciliation
• Perform physician-ordered specialized procedures
• Prepare equipment and materials using aseptic technique
• Complete documentation timely and accurately
