Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients’ homes. The role works under an RN’s supervision and focuses on coordinating with physicians, teaching caregivers, and ensuring accurate documentation.

Qualifications

• 1+ year of LPN or LVN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
