Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide quality home health care in Luzerne County. The role supports per-visit and full-time schedules, with an emphasis on coordinated care and infection control practices in patients’ homes.

Responsibilities include accurate charting, collaboration with clinicians, and flexible availability to meet patient needs. A valid LPN license and driver’s license with insurance are required, with CPR certification preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours to meet client needs.

Responsibilities

• Provides high quality clinical nursing services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates needs/changes to the team.
• Functioning well in diverse home settings and with varied patient needs.
