Job summary

Canyon Idaho Home Care and Hospice in Fruitland, ID is seeking a qualified Licensed Practical Nurse (LPN) to administer skilled nursing care in patients' homes on an intermittent basis, under physician orders and the plan of care, and supervised by an RN. The LPN will perform treatments, record observations, and support family education.

The role requires adherence to agency policies, effective communication with the clinical team, and timely documentation.

Qualifications

• Graduate of an approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
• Minimum of one year nursing experience preferred.
• Acceptance of the agency's philosophy and goals.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Furnishes services, treatments and diagnostic and preventive procedures per the plan of care.
• Prepares clinical and progress notes.
• Assists physician, intermediate care provider and RN in procedures.
• Prepares equipment and materials for treatments with aseptic technique.
• Assists patient in learning self-care techniques.
• Implements nursing care plan under RN direction.
• Initiates preventative and rehabilitative nursing procedures as directed by the RN.
• Observes signs and reports reactions to treatments to physician/RN.
• Teaches and counsels patient and family regarding nursing care needs.
• Maintains accurate patient records and participates in audits and QM activities.
• Attends staff meetings and care conferences.
• Delivers care within agency policies and maintains attendance.
