Job summary

Optum, part of the UnitedHealth Group family, is seeking a Licensed Practical Nurse for home health visits. You will provide skilled nursing care in patients' homes, following the plan of care and adhering to infection control procedures.

Responsibilities include documenting visits, collaborating with the care team, and coordinating with case managers to address patient needs. Flexible hours and a comprehensive benefits package are offered.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and the plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes in condition, and communicating with patient, family, physician, and other clinicians.
• Able to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
