Job summary

UnitedHealth Group in Nutley, NJ seeks a Licensed Practical Nurse in Home Health to provide quality patient care in patients' homes and support care coordination with the health team.

The role includes documenting visits, following the plan of care, and ensuring infection control procedures are followed. Compensation includes a mix of per visit point and hourly pay with full-time benefits.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Home care experience.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws.
• Documents patient visits thoroughly and in EMR per regulations.
• Participates in care coordination and communicates with patient, family, physician and team.
• Function in diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition.
