Job summary

All Ways Caring HomeCare is seeking a Licensed Practical Nurse (LPN/LPT/LVN) to provide direct and indirect care in a client’s home under RN supervision in Idaho Falls. The role emphasizes patient education, health maintenance, and safe home care practices.

Responsibilities include medication administration, accurate documentation, Start of Care assessments, and ongoing coordination with the care team to meet regulatory standards and client goals.

Qualifications

• Nursing degree from an accredited school.
• Experience administering medications.
• 1+ year of nursing experience preferred.
• Home health care experience preferred.
• Current and unrestricted nursing license in the state of operation.
• CPR certification.
• Strong oral, written, and interpersonal communication skills.

Responsibilities

• Provide skilled nursing care per Physicians orders and plan of care.
• Document nursing actions and client status; communicate with family and physicians.
• Medication administration and management as needed.
• Conduct Start of Care assessments in person where applicable.
• Complete supervisory visits and review plan of care and documentation.
• Coordinate with RN supervisor and Branch Manager to plan and evaluate services.
