Job summary

AdvisaCare is hiring a Home Health or Hospice RN in Michigan to provide compassionate, patient-centered care in a one-on-one setting. You’ll perform assessments, implement care plans, and collaborate with families and the care team to improve quality of life.

Qualifications

Current Michigan RN license in good standing.
Minimum 1 year home health and hospice experience.
Empathetic approach to patients and caregivers.

Responsibilities

Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
Implement and revise the individualized plan of care; ensure compliance with policies.
Educate patients and families on disease processes and self-care; document progress.
Supervise paraprofessionals; participate in IDG conferences and performance improvement.
Maintain high customer satisfaction; communicate using current processes and technology.
