Job summary

AdvisaCare Hospice is seeking an experienced RN or LPN to provide compassionate, in-home hospice care. This role emphasizes one-on-one patient support, family education, and collaboration with a multidisciplinary team in Michigan communities.

Responsibilities include supervising staff during visits, participating in IDG conferences, and maintaining detailed care plans and documentation to ensure dignified end-of-life care. On-call rotations are required.

Qualifications

• Current Michigan RN or LPN license in good standing.
• Minimum 1 year hospice care experience.
• Comfortable providing care in home settings.
• Ability to work full-time and participate in on-call rotations.
• Strong communication with patients, families, and care team.

Responsibilities

• Provide one-on-one hospice care in patients' homes.
• Conduct supervisory visits and clinical observation of staff.
• Participate in IDG conferences and update care plans.
• Educate patients and families on disease progression and end-of-life care.
• Maintain thorough documentation and progress notes.
• Maintain high patient/family satisfaction.
