Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Nurse Aide Training program.
• Current Nurse Aide certification in your state.
• At least 1 year of home health or hospice experience preferred.

Responsibilities

• Provide personal care to patients per the plan of care under licensed nurse supervision.
• Assist with daily activities: bathing, grooming, dressing, toileting, eating.
• Assist with walking, transfers, and positioning for safety and comfort.
• Measure and record vital signs as directed.
• Notice and report changes in patient condition to your supervising nurse.
• Document care accurately in our EMR and support care coordination.
