Job summary

Tender Care Home Health & Hospice is seeking a Registered Nurse to join our Las Cruces team, delivering compassionate, patient-centered home health and hospice services. The RN will assess, plan, and coordinate care in patients' homes within Southern New Mexico, working under a Clinical Supervisor with a strong focus on education, comfort, and quality of life.

Ideal candidates are motivated, compassionate, and prepared to lead care teams, educate families, and adhere to HIPAA and Medicare

Qualifications

• RN license required.
• Experience in home health setting preferred.
• Strong communication and teaching skills.
• Ability to coordinate with interdisciplinary teams.

Responsibilities

• Plan and direct home care services under Clinical Supervisor.
• Assess patients in their homes and manage pain, meds, and psychosocial needs.
• Develop individualized care plans with physicians, social workers, and therapists.
• Provide skilled nursing care including wound care and palliative measures.
• Educate patients and families on disease management and end-of-life care.
• Coordinate care transitions and document in EMR with HIPAA compliance.
