Job summary

CenterWell Home Health is seeking a Home Health Aide (HHA) to support patients at home under the supervision of an RN. You will follow the Plan of Care to provide direct care, monitor vitals, assist with activities of daily living and help patients regain independence.

Ideal candidates have a CNA or HHA credential within the last 24 months, meet state requirements, possess a valid driver's license, and have at least one year of HHA/CNA experience in healthcare settings.

Qualifications

• Must be certified as CNA or HHA within last 24 months.
• Must meet applicable state certification requirements.
• Valid driver''s license, auto insurance, and reliable transportation.
• In good standing on the HHA Registry (if applicable).
• Minimum 1 year experience as HHA or CNA in healthcare settings.

Responsibilities

• Provide direct patient care under RN direction and POC.
• Assist with self-administered medications and verify doses.
• Take vitals and report abnormal results.
• Support personal hygiene and grooming for patients.
• Assist with feeding and monitor nutrition intake.
• Assist with toileting and ostomy care as needed.
• Support mobility, transfers, turning and ambulation per Care Plan.
• Document care provided and communicate changes to team.
