Job summary

Humana Inc. is seeking a Home Health Aide for CenterWell Home Health in North Palm Beach County. You will provide in-home care to help patients regain strength and independence under RN supervision.

Responsibilities include vital signs, medication assistance, personal care, mobility support, nutrition and accurate documentation, with a commitment to infection control and safe home environments. A CNA/HHA credential and a valid driver license are required.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
• Must meet applicable state certification requirements
• A valid driver's license, auto insurance, and reliable transportation are required
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsibilities

• Provide direct patient care under RN supervision per the Plan of Care.
• Assist patient with self-administered medications and monitor dosing.
• Take vital signs and report abnormal results to supervisor.
• Help patient with personal hygiene including bathing, grooming and oral care.
• Assist in feeding and observe nutrition and intake.
• Support mobility, transfers and ambulation per Plan of Care.
• Record and document care interactions accurately for continuity.
• Practice infection control and maintain a clean, safe environment.
• Assist with light household tasks and meal preparation when needed.
