Job summary

Luxe Home Health LLC is seeking a dedicated Home Health Aide (HHA) to join our team serving Lake and Porter Counties on a per diem basis. You will assist with ADLs, monitor health, and maintain care plans while driving between patient homes.

The role requires at least 2 years of HHA experience, a valid Indiana driver's license, and current registration with the Indiana State aide registry. Travel across counties is expected to support patient care.

Qualifications

• 2+ years of experience as a Home Health Aide (HHA).
• Valid Indiana driver's license and reliable transportation.
• Travel within Lake and Porter Counties, Indiana.
• Registered with the Indiana State aide registry and in good standing.

Responsibilities

• Assist patients with ADLs including bathing, dressing, grooming.
• Provide light housekeeping and meal preparation in home care settings.
• Monitor patient health, document progress, and communicate with families.
