Job summary

Luxe Home Health LLC is seeking a dedicated Home Health Aide (HHA) to join our team in Lake and Porter Counties on a per diem basis. You will provide compassionate, high-quality care in patients' homes, assisting with ADLs, mobility, and daily tasks while following the established care plan.

The role requires at least 2 years of HHA experience, a valid Indiana driver’s license, and the ability to travel within Lake and Porter Counties.

Qualifications

• 2+ years of experience as a Home Health Aide (HHA).
• Valid Indiana driver’s license and reliable transportation.
• Ability to travel within Lake and Porter Counties, Indiana.

Responsibilities

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, meal prep, and light housekeeping.
• Monitor patient health and mobility per care plan.
• Document patient progress and communicate concerns to family and supervisors.
• Travel within service area to provide in-home care as needed.
