Job summary

PurposeCare is seeking compassionate Home Health Aide professionals to support patients in Ohio homes, delivering personal care, daily living assistance, and companionship. You will travel to client homes, follow care plans, and document activities while ensuring safety and comfort for clients.

Minimum qualifications include HHA training or equivalent; state certification per Ohio regulations; ability to pass background checks.

Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Home Health Aide (HHA) training program, CNA certificate, STNA license or at least 1.5 years of recent caregiving experience in home care, assisted living, long-term care, nursing home, or hospital settings.
• HHA certification or equivalent credential as required by Ohio state regulations.
• Basic knowledge of patient care and safety protocols.
• Ability to pass required background checks and health screenings.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (no independent med administration).
• Monitor and report patient condition and changes to the supervising nurse.
• Take and record basic vital signs when trained and assigned.
• Maintain a clean, safe, and orderly client environment.
• Assist with linens and light client laundry as appropriate.
• Use safety equipment properly (rails, supports, stools, etc.).
• Document care provided according to company and regulatory standards.
• Travel reliably to patient homes and assigned visits across your service area.
• Complete required in-service training, including annual dementia education.
