Job summary

Southern Ohio Medical Center is seeking a Home Care Aide to support patients in their homes under the supervision of the Nurse Manager. The position emphasizes care, comfort and safety, including vital signs, personal hygiene support and patient mobility during routine visits.

Education and licensing requirements include a high school diploma or equivalent, completion of a Home Care competency evaluation, and BLS within 90 days of hire.

Qualifications

• High school diploma or equivalent required.
• Home care competency evaluation program required.
• BLS within 90 days of hire required.
• BMVL driving eligibility required by SOMC insurers.
• Nursing assistant/home care training preferred.

Responsibilities

• Makes home visits and ensures patient care, comfort and safety under RN supervision.
• Performs basic patient care procedures such as vital signs and simple dressings.
• Assists transfers, ambulation, and ROM exercises as per Plan of Care.
• Performs light housekeeping and errands for patient comfort and safety.
• Observes and reports changes in patient condition to the Home Care Nurse.
• Documents care provided and observations to the nurse.
